2008-2009 Late Registration

All late registration for the Fall 2008/Spring 2008 Season will handled via mail only.
Due to limitations of roster size, late registrants are NOT guaranteed placement onto a team.
If placed, requests for a specfic team cannot be honored.
Fees :

$105.00 for 1st child	$90.00 for 2nd child
$20.00 for 3rd child	Free 4th child or more

Registration fee includes a $15 late fee for 1st child only as long as all children are registered at the same time. But registration fee does not include any uniform fee which must be arranged separately through the Uniform/Equipment Coordinator
Child must be born on or before July 31, 2004. NO EXCEPTIONS

Send the completed form along with the appropriate check made out to the West Islip Soccer Club and send to:
